Step 2: Nominate your beneficiaries
Superannuation law dictates who is eligible to receive a death benefit payment. When paying death benefits, the Trustee must make the
payment in such proportions as the Trustee decides to any one or more of the following persons:
* One or more of your dependants (spouse, defacto spouse, child, financial dependant, interdependent relationship dependent);
* Your legal personal representative (i.e. your estate);
Where no dependants can be identified, or the Trustee cannot establish that there is a legal personal respentative, the Trustee may distribute
the benefit to a person selected by the Trustee.
Please nominate in the table below your dependants and the proportion of your benefits to be allocated to each dependant. You can amend this
nomination at any time by notifying the Trustee through another ‘Nomination of Beneficiaries’ form.
When assessing who is entitled to your benefits in the event of your death, the Trustee will take this nomination into account, however is not
bound by this nomination.
